LSD in Sayamako

I feel tired when I woke up, but today was the last day of this year I could run.(I'm on the night duty tomorrow.) I decided that my last training menu was LSD and run through my favorite course "Hanakoganei-Tamako-Sayamako"
It was so called "Hashiri-Osame"?

In this year there were too many things that I never expected.
I appreciate all what I was given. And....

I regret a lot of my failure and many things that I did on my way.
Very sorry and please go aroud with me in the next year too.

ski

I woke up at 4:00 and went skiing with my staff to Kandatu where we can do Sunrise skiing. From 7:00 to 11:30 we skied very hardly so I had some muscle pain in both thighs.

Now I am writing New Year's Greetings. It takes much time...getting sleepy...

runner

At last I could "run" again, not jog.
When I was running as a kind of pace run,one runner talked to me while waiting a signal to change. Of course we have never spoken with each other, but have seen many times while running. He was belonged to Tama Racing Circle.

ski

We(my older daughter and I) went skiing for the first time in this year.Although it was raining in the morning, it was getting fine at noon. This year there is much snow than last year,so we can enjoy skiing much more in this year.

After going skiing, I let the hematoma punctured so the tension of the skin got much looser.

ganbattenai-papa

I was off for 9days. I'm afraid I can't run for more few days. My subcutaneous hemorrhage is descending and now reached to the right thigh. Pain is getting better but still here and there.]
Anyway I have to make a lecture about Triage on this Friday and have not yet finished the preparation. So in either case I have no time to run.

One point lecture" triage"
Triage is the process of rapidly and accurately evaluating patients to determine the extent of their injuries and the appropriate level of medical care required.
Priority is placed on providing care to victims most likely to survive; victims with such severe injuries that they are unlikely to survive are given low priority because they consume a disproportionate share of resources.
The most popular method is START(Simple Triage and Rapid Treatment).
START is an assessment of ambulation, respiration, perfusion, and mental status in a stepwise fashion to determine which victims require immediate transport for definitive care. START is easy to apply and designed for mass casualty incidents when time for comprehensive evaluation is nonexistent.

The system uses color-coded tags to identify patients and to designate their triage category: black is dead, red is critical, yellow is serious, green is not serious.

Anyway I want to run again.....

reading

I couldn't begin to run. Maybe it takes about 10 days to run again.
In spite of running,I read a book"博士の愛した数式". It was a one of best seller books and a few yeas ago a movie was made. It's worth reading it and I will try to see the movie.

ADL almost full

I was absent from job for two days. Because I was getting better and could walk around, my motive for running come out again. I have just applied to the Asahi Sakura Kenko Marathon(42.195km) held on 30th March. My target for this race is "sub three" to finish the race within three hours! But, I wonder when I'll be able to run again....

Off course, off

I'm in my house all day long. With self examination, reflecting on my past conduct.

But,.....I can't give up mountaineering race.

Mitake-san trail run: 15km
monthly mileage for December 45m

Mitake-san Trail run, got injured...

Recently trail run has become popular and there was over 900 entries to this race.

And this race in the last year was the first mountaineering race that I participated. I wanted to see the different "me" from that of last year.
As soon as the start I paced up,and kept high pace. Most of the first half is upslope, and then we dashed the latter downslope. To tell the truth, I had been getting much more confidence in downslope in comparison to last year. I paced up more and passed a few runners!!
In the steep downslope a root of a tree caught my foot, and fell down(just like "dive into the bottom world"). I was hit upper the right edge of pelvis. For a while I couldn't move from there, but shortly I could begin to walk and then jog slowly. Many runners passed me and saying " You fell down? Are you alright?" and I replied "I'm fine, thank you" but in my heart I shouted "never alright, couldn't regret more...butu-butu-butu-butu".I wonder whether I should continue the race or not, but there may be last 3km left so I kept running(jogging) and could reach the goal.

A tragedy came after that. My back pain got worse and worse. The pain was like that of burn or something like electric. Maybe the hematoma became bigger and the mass compressed the nerve. Finally, from the cable car station I couldn't walk any more(couldn't keep standing). The station staff carried me on his back and took me into my car. My wife drove the car and took me to my hospital. No fracture, and great hematoma on my right back. I was laughingstock in my hospital. I learned a new thing. A bruise can make a terrible pain derived from acute nerve compression and skin expansion.

great appreciation to my wife,my daughters, station staffs and hospital staffs.

Mitake-san Trail run, everything is ready!!

Yesterday Mitake-san trail run was held. It was only 15km, so-called a splint mountaineering race. My cold had got well and according the weather forecast it'll be very fine. Best situation for the race (except my short of training...).
After taking the Mitake cable car, we stayed Kuraya.

Although it rained and hailed much the previous night instead of the forecast, I could see the wonderful red sky, a mourning glow while mourning walking.

Saturday, December 8, 2007

I will attend the Mitake-San mountaineering race tomorrow and from now go to Mitake and stay there tonight. It'll be fine tomorrow so the race will be pleasant and refreshing.

Recently I can see ice needles on my way to the hospital. In such a cold day Amazake is the best drink to make yourself warm. You should choose non-sugar Amazake and take much of clustered ginger into it.

The Last month of 2007

At last month my cold hadn't got well clearly. It was a kind of "a chronic cold"? So this month I will care of my health much more, to put it concretely, not to wear too may clothes, to have enough gargle, to keep warm after running and so on.

Christmas season has already come to Shinjyuku.

The illumination was fantastic but it was too crowded.

The first step

We my family enjoyed hiking around Tominnomori in Hinohara-mura.This was the first step for my daughters to begin moutaineering... It was a little late for the red and yellow autumn leaves but we could see icicles in the river and many frost flowers.
So we could experience both autumn and winter scenery.

The pass was steeper than I thought.

One of icicles in the river

dried persimmons Look very sweet but we couldn't eat them
This Dango is the reward for us who kept it up.

good couple day

It is extremely cold. And there is another reason not to run?????
My wife went for a drink with her friends(a year-end party), so my daughters and I ate out.
By the way, in Japan, today is the day for good married couple. (you can read 11-22 as "i-i-hu-hu"). I deeply appreciate all what my wife and daughters give me.

Happy birthday

Today is my younger daughter's birthday.(but it's busy today so we held the birthday party last Saturday.) Her present was Korirakkuma?

Last night it was a little hard because a Pt of Lithium poisoning came in 1:00 am and we had to care her till morning (include hemodialysis). I fell asleep just after coming home. So I couldn't make an enough time to run.

Today's training: 8km run
monthly mileage for November 134km

Today's one point emergency medicine(from today I'll begin one point lecture of ER!!)
" Lithium poisoning"
Lithium carbonate is an effective drug in the treatment of manic depressive illness. However, it may induce multisystem dysfunction which, if not recognized, may be fatal. Findings that may be observed include neuromuscular disorder, seizures, coma and so on. Many text book tell that the plasma lithium concentration is a good parameter of clinical course and treatment, but in most hospitals like us they can't get the result of the plasma concentration timely. So we have to decide how to treat it in terms of clinical presentation. The most difficult point is to decide whether to do Hemodialysis or not. Lithium is the most dialyzable toxin known.

Mitake-san Trail running

It was a little chilly but fine and good for trail.
Jumping into the 6:11 train and began to run from Musashiitukaichi station to Mitake San and enjoy the autumn scenery around the Mitake-San and then returned to Musashiitukaichi again.

They said leaves didn't turn red beautifully in this year.

The water of the waterfall was very cold!!!
Off course I didn't jump into the waterfall.

Oshiruko made in Mitake-san. Sweet gave me the power to run back to the Musashiitukaichi.

Bad day or Good day?

Cold wind took my running motivation, but it was also the best situation to try my new windbreaker!!!

Yesterday a twenty seventh years old young man fell down with ventricular fibrillation "Vf" while playing football with his friends at a gymnasium. Immediately one of his friends started CPR and the other brought an AED and delivered shock two times and got the recovery of spontaneous circulation.
Soon after the emergency medical service "EMS" arrived and dispatched him to our hospital. while dispatch, he fell down several times and was delivered three times shock by EMS.
Finally his cardiac rhythm was Vf on arrival,so we continued to deliver DC (total was four times) and give several drugs for arrhythmia. At last we got his stability and he had no neurological deficit!!!
We could talk with him about his friends ,fiance and so on.
Great by standers, great EMS staffs and great our ER team!!? It was a GOOD day!

New shoes!

I bought two running shoes last Tuesday as training ones those had soft soles but weight a little heavily. Both were Asics(I forgot the name..GelXXXX?) and not so bad,but I thought Mizuno "Wave Rider" was the one of the best shoes for training.
Why not bought Mizuno? Because I like Asics "Gel Fuji" and to tell the truth I chose the cheaper ones...I regret a little...

Finally we(include grandparents)had late lunch at a restaurant. It was a long day...

Shichi-go-san is a Japanese traditional festival to celebrate the growth of children. Boys of five, girls of seven and three-year-old boys and girls will be taken to a shrine by their parents in order to give thanks to God for His help.

Mt Kumotori and Mt Fuji

I woke up at 4:00 am and went to Okutama. The bus was full of hikers and most of them were the 50's or 60's(?). Their power was admirable! I climbed up Mt Kumotori at first where we could see the marvelous Mt Fuji, then run along the mountain ridges. It was a good season to see the autumn scenery, such as colorful fallen leaves. Finally today's goal was Moeginoyu(Onsen).

I knew the news from internet that 40 years old male runner slipped the valley wall and died at the 45km point near Gozen-Yama.

And we have to think about security of trail running ourselves again.

Japan Moutaineering Race “Hasetune Cup”

I finished the race!!! And felt great satisfaction
The time: 10:14
Finished the race in 51st.(more than 2300 runners entried and 2003 runners started)

Here is my race report.

We could see the blue sky and feel comfortable if we don’t run! It was too fine and the temparture was 24℃. To meet the situation, I took 2.75L of drink and consequently my bag weighed 4.7kg.
The strategy of this time was that in the first half I drink enough and save my stamina and run faster after my bag gets lighter.
The cource condition was not so good because of the yesterday’s rain. It was muddy, foggy and easy to slip. (I slipped and fell more than five times.)
But actually my condition was very good and I realized it after I passed the first chek point(22.6km).
Soon after I passed the first check point, it was getting dark so I put my lamp on. Till the middle stage there were some runner in front and behind me, but between the last 30km, there were few runner around me and it was complete dark. While running, I could find the small light ahead and it came nearer and neaer and then I passed by him. After that the dark came again.
Although I lost the way once,I passed by many runner in the latter half and could finished with a smiling face.
Surely I will be back in the next year! It was one of the best experience that I have ever had.

A photo before start.
